Surelog is licensed to you under Apple's Standard Licensed Application End User License Agreement, together with the terms below.

Not medical advice

Surelog is a personal record-keeping tool. It does not diagnose, treat, cure or prevent any condition, and it does not calculate, adjust or recommend doses. Doses, schedules and lab values are shown as you enter them. Always follow your prescriber's instructions and contact a health professional with any questions about your treatment.

Reminders

Reminders depend on your device's notification settings and may not always be delivered. Do not rely on Surelog as your only way of remembering a dose.

Subscriptions

Surelog Premium is available as an auto-renewing subscription (monthly or yearly, with a free trial where offered) or a one-time lifetime purchase. Payment is charged to your Apple ID. Subscriptions renew automatically unless cancelled at least 24 hours before the end of the current period. You can manage or cancel in Settings › Apple ID › Subscriptions.

Your data

You are responsible for your device and backups. We are not liable for data lost through device loss, deletion of the app or forgotten backup passphrases.

Liability

To the extent permitted by law, the app is provided "as is" and PitchCall, Inc. is not liable for indirect or consequential damages arising from its use.
